Job summary

Renalogic, Inc. seeks a Clinical Nurse Coach to provide remote health coaching to members at risk for chronic kidney disease. You will work with members to improve understanding and management of kidney health, using NCQA guidelines and motivational interviewing.

This full-time, remote role requires PST hours with occasional evenings and weekends. An RN license with five years of experience is required, BSN preferred, and travel may be needed for corporate events.

Qualifications

• RN license in good standing
• Five years RN experience
• Willing to work PST hours including evenings/weekends
• Experience developing care plans for chronic conditions
• BSN preferred
• CDE or CDCES preferred
• Remote nurse experience preferred
• Ability to travel occasionally
• Strong communication skills

Responsibilities

• Establish contact with members via video, phone, text, and email in English
• Develop individualized member care plans per NCQA guidelines
• Assess health status and care coordination needs
• Collaborate with members' health care providers as needed
• Utilize motivational interviewing and Triple Aim to empower members
• Explain program benefits to new/inactive members
• Document and share member information in HIPAA-compliant manner
• Meet performance metrics for member engagement and retention
