Job summary

Americare is seeking an Intake Coordinator who is a Registered Nurse to assess referrals and coordinate appropriate home care services. You will maintain clear communication with referral sources and across Americare departments, ensuring high-quality, patient-focused service.

Responsibilities include processing referrals, collaborating with CHHA teams, and promoting Americare through exemplary customer service.

Qualifications

• Current NYS RN License is required.
• Allscripts experience is required.
• CHHA experience is required.
• Computer literacy; experience with Home Care Home Base is required.
• Excellent communication skills are essential.
• Highly organized with strong attention to detail.

Responsibilities

• Takes referrals and processes appropriate home care services.
• Maintains open communication with referral sources and Americare departments.
• Promotes Americare through positive customer service.
