Job summary

Vaya Health is seeking a Registered Nurse to perform needs-based assessments for State Plan Personal Care Services in beneficiaries' homes and community settings within North Carolina. The RN will schedule and conduct telephonic and face‑to‑face assessments, collaborate with healthcare professionals, and submit SARs for PCS providers.

Required is a NC RN license with a bachelor’s degree in nursing or a related field and at least two years in home care.

Qualifications

• Knowledge of NC Medicaid Clinical Coverage Policy and PCS provider manuals.
• Understanding of HCBS programs, EPSDT, and public sector services.
• Proficient with Microsoft Excel, Word and Outlook.
• Ability to interact with healthcare professionals, patients and families.
• Strong attention to detail and the ability to work independently.

Responsibilities

• Schedule assessments with members, families and other providers.
• Assess PCS eligibility using standardized tools.
• Interview family members and caregivers present at assessments.
• Ensure assessments occur within established timeframes.
• Provide education to select PCS providers and assist in decisions.
• Coordinate with prescribers to ensure requests are complete and accurate.
• Submit SARs on behalf of PCS providers.
• Maintain privacy and dignity of individuals assessed.
• Participate in mediation/appeals and respond to state inquiries.
• Attend staff meetings and participate in case consultations/audits.
