Vaya Partners seeks a Registered Nurse to perform needs-based assessments for Medicaid Personal Care Services in NC. You will schedule and conduct telephonic and in-home assessments, work with families and healthcare professionals, and submit SARs for PCS providers.

The role requires NC RN licensure, a healthcare background, and the ability to travel within Alamance, Chatham, and Caswell counties. Remote work with NC residency is required and open until filled.

Qualificações

• Bachelor's degree in Nursing or healthcare field required; two years in home care preferred.
• Experience conducting PCS assessments preferred.
• Must have active RN licensure in North Carolina.

Responsabilidades

• Schedule assessments with members, families, and healthcare professionals.
• Conduct needs-based PCS assessments to determine eligibility.
• Interview family members and informal caregivers.
• Ensure assessments are completed within established timeframes; conduct expedited assessments by phone as needed.
• Submit SARs on behalf of PCS providers; coordinate with prescribers and other professionals.
• Maintain privacy and dignity of beneficiaries; participate in mediation/appeals as needed.
• Respond to state inquiries regarding assessments.
