Vaya Partners is seeking an experienced Registered Nurse to perform needs-based assessments for State Plan Personal Care Services in beneficiaries' homes or community settings. You will schedule and conduct telephonic and face-to-face assessments, collaborate with healthcare professionals, and submit SARs on behalf of PCS providers.

Qualified candidates hold a NC RN license and a Bachelor's degree in Nursing, with 2 years of home care or PCS assessment experience preferred.

Competenze

• Bachelor's degree in Nursing or other healthcare field required.
• Active North Carolina RN license required.
• Two years of home care experience preferred; PCS assessments experience valued.

Mansioni

• Schedule assessments with members, families and other professionals.
• Conduct needs-based PCS eligibility assessments using standard tools.
• Perform in-home and telephonic assessments; coordinate with providers.
• Submit SARs for PCS providers and ensure accurate documentation.
• Protect privacy and dignity of beneficiaries; participate in internal reviews.
