Job summary

Vaya Partners in North Carolina is seeking a Registered Nurse to perform needs-based assessments for State Plan Personal Care Services in home and community settings. The PCS Assessor will schedule visits, conduct telephonic and in-person assessments, and work with providers to determine eligibility.

The role requires NC RN licensure, a bachelor’s degree in nursing or equivalent, and at least two years’ home care experience.

Qualifications

• Bachelor's degree in Nursing or equivalent health care field.
• Active North Carolina RN licensure.
• 2+ years in home care or PCS assessments preferred.

Responsibilities

• Schedule and conduct PCS assessments in members' homes.
• Determine PCS eligibility using standardized assessment tools.
• Submit SARs on behalf of PCS providers and coordinate with prescribers.
• Maintain privacy and dignity of clients; adhere to Medicaid policies.
• Participate in staff meetings and case consultations as needed.
