Job summary

The Personal Care Services (PCS) Assessor Registered Nurse at Vaya Health conducts needs-based assessments for Medicaid beneficiaries applying for State Plan PCS in home and community settings across designated NC counties. You will schedule visits, perform telephonic and in-person assessments, and submit SARs while protecting patient privacy.

Qualified candidates hold a NC RN license, a Bachelor's degree in Nursing or related field, and experience in home care or community-based settings;

Qualifications

• Bachelor's degree in Nursing or healthcare field required.
• Active RN licensure in North Carolina.
• Experience in home care or community-based settings preferred.

Responsibilities

• Schedule assessments with members and families.
• Conduct assessments to determine PCS eligibility using standardized tools.
• Submit SARs on behalf of PCS providers.
• Maintain privacy and dignity of individuals receiving assessment for PCS.
• Coordinate with prescribers and other healthcare professionals.
• Respond to state inquiries regarding assessments.
