UnitedHealth Group is seeking a Discharge Care Manager to implement telephonic case management for high-risk members. You will assess needs, coordinate discharge planning with clinical teams, and support post-hospital care in collaboration with providers.

The role supports remote work for EST/CST time zones, with ongoing collaboration in cross-functional care teams to improve health outcomes. Nursing licensure and hospital experience are required.

Qualifications

• Bachelor of Science in Nursing and an unrestricted RN license.
• 5+ years of recent inpatient hospital experience.
• 2+ years of discharge planning and/or case management.
• Proficient in typing and software applications including Word/Excel/PowerPoint/Outlook.

Responsabilités

• Assess discharge needs and coordinate post-discharge care.
• Collaborate with providers and members to arrange follow-up services.
• Participate in rounds with Medical Director to discuss cases.
• Document discharge activities and data entry per SOPs.
