Job summary

CVS Health is hiring a Nurse Case Manager (RN) for a field, work-from-home role with frequent local travel across Dallas area Texas. You will assess and manage member healthcare needs via telephonic and in-person visits, developing care plans and coordinating services to improve outcomes.

The role requires an active RN license plus BSN, with 3+ years clinical experience and at least 1 year in case management.

Qualifications

• Active, unrestricted RN license.
• BSN degree required.
• Minimum 3 years of clinical experience.
• At least 1 year of case management experience.
• Bilingual English/Spanish preferred.

Responsibilities

• Conduct comprehensive telephonic and in-person assessments of members.
• Develop and implement individualized care plans aligned with clinical guidelines.
• Coordinate and monitor care plan activities to improve outcomes.
• Collaborate in multidisciplinary reviews to optimize member health results.
• Identify and address quality of care concerns through proper channels.
• Engage members with motivational interviewing to promote healthy choices.
• Educate members to participate in healthcare decisions with providers.
• Document accurately adhering to regulatory and company standards.
• Serve as a preceptor and mentor to new nurses and clinical staff.
