Job summary

CVS Health is seeking a Nurse Case Manager (RN) for a field-based, work-from-home role that requires frequent local travel across Dallas and surrounding Texas communities. This full-time position supports in-home member visits and coordination of care with providers to improve health outcomes while maintaining flexibility.

You will conduct assessments, develop care plans, monitor progress, mentor staff, document accurately, and collaborate with multidisciplinary teams.

Qualifications

• Active, unrestricted RN license.
• BSN degree required.
• 3+ years clinical experience.
• 1+ year case management experience.

Responsibilities

• Conduct comprehensive telephonic and in-person assessments to evaluate members' medical and behavioral health needs.
• Develop and implement individualized care plans aligned with clinical guidelines.
• Coordinate and monitor ongoing care plan activities and adjust to improve outcomes.
• Collaborate in multidisciplinary care reviews to optimize member health results.
• Identify and elevate quality of care concerns through appropriate channels.
• Engage members using motivational interviewing to encourage healthy lifestyle choices.
• Educate and empower members to participate in healthcare decisions with providers.
• Use negotiation and coordination skills to secure services and resources.
• Maintain accurate documentation adhering to regulatory and company standards.
• Serve as a preceptor and mentor to new nurses and clinical staff.
