Job summary

CVS Health is seeking a Nurse Case Manager (RN) for a remote role with frequent local travel across the Dallas & Surrounding Areas in Texas. This work-from-home position combines telephonic and in-person member visits to deliver high-quality case management and improve health outcomes.

You will assess medical and behavioral health needs, develop care plans, coordinate services, mentor staff, and ensure compliant documentation, while maintaining safety and quality in every interaction.

Qualifications

• Active, unrestricted RN license.
• Bachelor's degree in Nursing (BSN).
• Minimum 3+ years of clinical experience.
• At least 1+ years of case management experience.

Responsibilities

• Conduct comprehensive telephonic and in-person assessments.
• Develop and implement individualized care plans.
• Coordinate and monitor ongoing care activities.
• Collaborate in multidisciplinary care reviews.
• Educate and empower members in decision-making with providers.
• Ensure accurate documentation and regulatory compliance.
