Job summary

Molina Healthcare is hiring a licensed RN to serve as Case Manager for pregnant members, focusing on high-risk cases, primarily via telework. You will assess needs, coordinate services, and guide members toward desired outcomes across LTSS and waiver programs.

Ideal candidates have 2+ years in health care with care management experience, strong MS Office skills, and the ability to travel periodically. Work hours are 8 AM–5 PM CST, remote from home.

Qualifications

• RN license active and unrestricted in state of practice.
• 2+ years healthcare experience incl. care management.
• Experience with LTSS/waiver services preferred.
• Ability to travel 25–40% as required.

Responsibilities

• Complete comprehensive member assessments within regulated timelines.
• Develop and implement care plans with members and care network.
• Monitor care plan effectiveness and document interventions.
• Coordinate care across behavioral health and LTSS.
• Identify barriers and coordinate resources for members.
