Job summary

Sentara Health in Virginia Beach seeks a Remote Integrated Case Manager to coordinate care for members across the continuum. RN licensed, with 3 years nursing experience; requires Associates, BSN preferred.

Role includes telephonic and face-to-face assessments, care plan development and coordination with physicians and care teams; remote role for Virginia residents; competitive compensation and full benefits.

Qualifications

• Associates degree is required.
• RN license is required.
• 3 years nursing experience required.
• Disease management and managed care experience preferred.

Responsibilities

• Provide case management services within licensure scope and develop/monitor/revise care plans to optimize health care across the continuum.
• Perform telephonic and face-to-face clinical assessments as needed for identification, evaluation, coordination and management of member needs.
• Identify high-risk members and coordinate with the care team to manage chronic illnesses and disabilities.
• Facilitate member care plan implementations by coordinating authorizations/referrals within benefits or external arrangements.
• Interface with Medical Directors and Inter-Disciplinary Teams on treatment plans and present cases at conferences.
• Ensure compliance with regulatory and company policies.
