Job summary

Sentara Health in Richmond, VA is hiring an Integrated Case Manager, RN to coordinate patient care across the continuum. This remote-friendly, full-time role requires in-person assessments and travel to Winchester, Leesburg, Alexandria, and other sites.

You will develop and monitor care plans, perform telephonic assessments, and collaborate with medical directors and care teams to optimize outcomes. An RN license is required with 3 years of nursing experience; managed care and discharge planning

Qualifications

• Registered Nurse licensure required.
• 3 years of nursing experience required.
• Managed care experience preferred.
• Discharge planning experience preferred.

Responsibilities

• Provide case management within licensure scope and monitor care plans.
• Perform telephonic clinical assessments for care coordination.
• Identify high-risk members and coordinate with health care teams.
• Present cases at multidisciplinary case conferences and ensure regulatory compliance.
• Travel for in-person assessments to designated sites as needed.
