Sentara Health Administration, Inc. is seeking a Remote Integrated Case Manager in Virginia Beach, VA. The role conducts telephonic and in-person assessments, develops care plans, and coordinates care across physical and behavioral health, social services, and long-term care needs.

The ideal candidate has an Associates degree (RN) with BSN preferred, and at least 3 years of nursing experience. This remote position supports multiple states and collaborates with medical directors and care teams

Qualifications

• Associates degree required.
• BSN preferred.
• 3 years nursing experience required.
• Registered Nurse certification required.

Responsibilities

• Perform telephonic and face-to-face clinical assessments for identification, evaluation and management of member needs.
• Develop, monitor, and revise member care plans across the care continuum to optimize health outcomes.
• Coordinate authorizations/referrals within benefits structure or extra-contractual arrangements as permissible.
