Job summary

Sentara Health Plans is seeking an Integrated Nurse Case Manager/Registered Nurse for Chronic/Complex Condition to support members in Hampton, Newport News, Yorktown, and surrounding areas. This is a full-time, work-from-home role with travel for monthly home visits (minimum 12).

Ideal candidates have an RN license (compact or VA), Associate or Bachelor's in Nursing, at least 3 years of nursing and case management experience, and strong communication skills.

Qualifications

• Associate or Bachelor’s Degree in Nursing is required.
• 3 years of Nursing experience required; Case Management experience preferred.
• Licensed Registered Nurse (VA or Compact) required.
• Experience with chronic/complex condition members preferred.
• Strong communication and analytical skills required.

Responsibilities

• Case management services within licensure scope; develop and revise care plans to optimize health care across the continuum.
• Perform telephonic or face-to-face clinical assessments for identification, evaluation, coordination and management of member needs.
• Identify high‑risk members and coordinate care with the health care team.
• Manage chronic illnesses and co-morbidities ensuring cost-effective health benefits use.
• Assist with care plan implementation by facilitating authorizations/referrals.
• Interface with Medical Directors/Physician Advisors on care plans.
• Present cases at case conferences; ensure regulatory and policy compliance.
• Assist with problem solving for provider, claims, or service issues.
