Job summary

Five Stars Care LLC is seeking a dedicated RN/Case Manager Nurse to join our growing home care team in Northern Virginia. This full-time/part-time role focuses on completing client assessments and reassessments to ensure high-quality, individualized care for patients in their homes.

The position emphasizes in-home assessments, care planning, and adherence to care standards, with flexible scheduling and options for paid mileage for eligible travel.

Qualifications

• Must hold a current, unrestricted Registered Nurse (RN) license in the Commonwealth of Virginia.
• Experience conducting in-home clinical assessments and completing Medicaid DMAS Forms 97, 98, 99, and 100.
• Ability to work independently.

Responsibilities

• Conduct initial client assessments and routine reassessments in client home settings.
• Conduct hospital discharge assessments in client home settings.
• Develop and update individualized care plans.
• Ensure compliance with care standards.
