ComForCare Home Health Care in Arlington, VA is seeking an experienced RN Case Manager (Assessment Only) to conduct thorough in-home assessments and manage OASIS and start-of-care evaluations. You will collaborate with physicians, care teams, and families to develop individualized care plans and ensure regulatory compliance.

The role requires strong clinical assessment, communication, and organizational skills, with a flexible schedule for field-based visits in the Arlington area.

Qualifications

• Active and unrestricted RN license in Virginia.
• Proficiency with OASIS documentation and Medicare home health regulations.
• Strong clinical assessment, critical thinking, and communication skills.

Responsibilities

• Conduct comprehensive in-home patient assessments to determine care needs and eligibility for home health services
• Complete OASIS assessments, start-of-care evaluations, and recertification assessments in compliance with regulatory standards
• Collaborate with physicians, care teams, and family members to develop and communicate individualized care plans
• Document clinical findings accurately and in a timely manner using electronic health records (EHR)
• Ensure compliance with Medicare, Medicaid, and Joint Commission guidelines during all assessment activities
• Communicate patient status changes and clinical concerns to the appropriate care team members
• Maintain a flexible schedule to accommodate patient and agency assessment needs across the Arlington, VA service area
