Job summary

Ashealthnet seeks a Registered Nurse Case Manager to lead patient assessments and care plans in a home health setting. You will coordinate multi-disciplinary teams, update physician orders, and ensure quality, cost-effective care under physician direction.

Qualifications

• RN with current license in the state of employment.
• Minimum of one year of RN experience in an acute care setting; home care experience preferred.
• Excellent communication and interpersonal skills; ability to educate patients and families.
• Detail-oriented with strong documentation and adherence to guidelines.

Responsibilities

• Oversees patient care from admission to end of episode and coordinates all disciplines involved.
• Updates physician orders and notifies changes in patient condition.
• Conducts OASIS assessments to develop an individualized plan of care.
• Provides skilled nursing care at the patient’s home as directed by the attending physician.
• Educates patients and families on disease management and preventive care.
• Ensures documentation complies with Medicare and federal guidelines.
