Job summary

Sentara Health is seeking an Integrated Case Manager, RN for a full-time daytime role. The position is remote but requires in-person face-to-face assessments with travel to King George, Tappahannock, Doswell, Ashland, Bowling Green, and other sites.

The candidate will conduct telephonic and in-person assessments, develop and manage care plans, coordinate with the care team, and ensure quality and regulatory compliance.

Qualifications

• RN with licensure required.
• 3 years of nursing experience required.
• Managed care experience preferred.

Responsibilities

• Perform telephonic and in-person case management assessments.
• Develop, monitor, evaluate, and revise the member care plan across the care continuum.
• Coordinate care with health care team and authorize referrals as needed.
• Identify high-risk members and coordinate chronic illness management.
• Present cases at multidisciplinary case conferences and ensure regulatory compliance.
• Assist in problem solving with providers, claims, or service issues.
