Qualifications

Responsibilities

• Telephonic case manager with patients and care teams for fully and/or self-insured clients.
• Apply criteria, clinical guidelines, and policies to assess benefits and member needs.
• Use clinical judgement to reduce risk factors in care planning.
• Assess information from various sources to address comorbid conditions.
• Consult with supervisors and others to overcome barriers and meet goals.
