Job summary

CVS Health is hiring a Case Manager Registered Nurse – Field to develop and implement care plans for members, coordinating with providers and families. This role requires strong clinical judgment for ML TSS/ D-SNP programs, and involves significant field travel in Union and surrounding counties.

The ideal candidate holds an NJ RN license with 3+ years of clinical experience, residency in Union County, and the ability to work 40 hours per week with remote work integration as needed.

Qualifications

• Active and Unrestricted RN license in NJ.
• 3+ years clinical practice experience.
• Must reside in Union County, NJ and travel 50-75% within NJ.

Responsibilities

• Conduct face-to-face home visits using assessment tools for MLTSS/D-SNP and non-MLTSS members.
• Develop care plans, coordinate with providers, and document in the EHR.
• Coordinate and advocate for appropriate services and safe discharges.
• Mentor new hires after proficiency is attained.
