Qualifications

• Must reside in Essex County, NJ.
• 3+ years of clinical practice experience (hospital or equivalent).
• Active NJ RN license.
• Willingness to travel up to 75% across Essex County and surrounding counties. NJ Choice certification required as contingency of employment.

Responsibilities

• Conduct face-to-face home visits for assessments and care planning under MLTSS/D-SNP programs.
• Coordinate with care team to develop and authorize cost-effective service plans.
• Document accurately in the member's electronic health record.
• Schedule and participate in interdisciplinary team meetings and advocate for safe discharge.
• Mentor new hires after achieving proficiency.
