Job summary

CVS Health in New Jersey is seeking an RN with strong case management to join our Community Care team. You will coordinate care for chronically ill members, liaise with providers, insurers, and families, and help connect patients with community resources to improve outcomes.

The role requires a NJ RN license, an Associate’s Degree in Nursing, and 3+ years of clinical experience with 2+ years in case management, with travel up to 10%.

Qualifications

RN license active and unrestricted in New Jersey.
Associate's Degree in Nursing required; Bachelor’s preferred.
Minimum 3+ years of clinical practice and 2+ years in case management or discharge planning.

Responsibilities

Act as liaison with members, providers, insurers, and healthcare personnel.
Coordinate case management for catastrophic/chronically ill members across the continuum of care.
Interact with members telephonically or in person; conduct home visits as needed.
Assess medical and vocational status; develop care plans to optimize wellness and outcomes.
Document all case work activities and communicate with multidisciplinary teams.
