Qualifications

• Associate degree required; Bachelor’s degree preferred.
• Active NYS RN or Registered Dietitian or Physical Therapist licensure required.
• Minimum of three years of clinical experience.
• Case Management experience preferred.
• Proficiency with Microsoft Office Suite.

Responsibilities

• Manage case management programs per department policies and standards.
• Screen and assess members; develop care plans and provide education and support.
• Collaborate with interdisciplinary care team and community resources to coordinate services.
• Ensure compliance with NYS DOH, CMS regulations and NCQA accreditation standards.
• Mentor and support junior staff; participate in orientation and training.
• Maintain confidentiality and quality of information management across processes.
