Job summary

Central Health seeks a Nurse Case Manager to identify, screen, track and coordinate care for members with multiple co-morbidities and psychosocial needs. This role may work remotely with occasional travel to Austin, TX, under the Manager of Case Management.

The ideal candidate will hold an RN license, have 3 years in community health or case management, and possess strong communication, documentation, and collaboration with providers across disciplines.

Qualifications

• Active and unrestricted RN license in good standing.
• 3 years of experience in community health, public health, chronic disease management or case management.
• Experience coordinating care with primary care providers.
• Strong written and verbal communication and documentation skills.
• Knowledge of case management, disease management and chronic care principles.

Responsibilities

• Complete a comprehensive physical, medical, and psychosocial assessment on high risk patients via telephonic interview.
• Establish care plans, goals, interventions and contact schedules based on risk category and identified needs.
• Provide self-management education materials and supports including disease education, nutrition, exercise and monitoring.
• Coordinate care and communication between multiple providers and services.
• Must be able to independently travel off-site to various locations.
