Job summary

CVS Health is seeking a Case Manager RN for a 100% remote role. You will assess, plan and coordinate care to meet members’ benefit plans and health needs, with a focus on advocacy, cost-effective outcomes and collaboration with Medical Directors as needed.

The role requires an active RN license in the state of residence and 3+ years of acute care experience. Regular hours on weekdays with some occasional extended shifts, and promotions toward expanded licensure are included.

Qualifications

• Active RN license in state of residence.
• 3+ years of acute care RN experience.
• Willingness to obtain additional state licenses upon hire.
• Must be able to work Monday–Friday 8:00am–4:30pm in residence time zone and occasional late shift.

Responsibilities

• Apply data driven methods to identify members for case management.
• Conduct comprehensive clinical assessments.
• Evaluate needs and develop flexible approaches based on plans or external programs.
• Advocate for patients to maximize coverage.
• Promote quality and cost‑effective outcomes; suggest program improvements.
• Identify and elevate care issues through established channels.
• Provide high level of customer service to members.
• Use clinical assessment to determine health literacy and readiness to change.
• Use motivational interviewing to engage members.
• Provide coaching and education to empower independent health decisions.
