Job summary

CVS Health is seeking an RN Case Manager to elevate patient care using best-in-class operating and clinical models. This remote role in New Jersey offers a Monday–Friday schedule (8:00 AM–5:00 PM ET) and a pay range of $66,575–$142,576 per year.

You will coordinate care across members, providers, and healthcare personnel, with opportunities to impact community care. Education required includes an Associate’s Degree in Nursing, with a Bachelor’s preferred.

Qualifications

• 3+ years clinical practice experience.
• 2+ years case management, discharge planning, and/or home health care coordination experience.
• Active and unrestricted New Jersey RN Compact License required

Responsibilities

• Act as a liaison with members, providers, and healthcare personnel.
• Implement and coordinate case management activities for catastrophic and chronically ill members.
• Interact with members/clients telephonically or in person.
• Assess and analyze medical and vocational status of members/clients.
• Develop a plan of care to optimize wellness and medical outcomes.
• Communicate with stakeholders regarding care and treatment.
• Prepare documentation of case work activities.
• Consult with internal multidisciplinary teams for best health outcomes.
• Provide educational and prevention information for optimal medical outcomes.
• Monitor member/client progress toward desired outcomes.
