Job summary

Sentara Health Plans is hiring an Integrated Nurse Case Manager/Registered Nurse (RN) for chronic/complex conditions in Fredericksburg, VA, with travel to surrounding areas. This is a full-time, work-from-home role requiring 2–3 face-to-face home visits per week.

Ideal candidates hold an RN license (VA or compact) and have 3+ years of nursing experience, including case management. Associate or bachelor’s degree in nursing is required.

Qualifications

• Associate or Bachelor’s Degree in Nursing required.
• Minimum of 3 years nursing experience with case management.
• Experience with chronic/complex conditions preferred.
• Licensed Registered Nurse (RN) in VA or compact state required.

Responsibilities

• Develop, monitor, evaluate and revise the member care plan to optimize health outcomes.
• Conduct telephonic or face-to-face assessments for physical/behavioral health and social services needs.
• Identify high-risk members and coordinate care with the care team.
• Manage chronic illnesses and ensure cost-effective utilization of benefits.
• Assist with care plan authorizations/referrals within benefits or contracts.
