Job summary

CVS Health is seeking a Case Manager RN to deliver remote care coordination across states. You will assess, plan, advocate and coordinate services to meet members’ benefit and health needs through available resources, while ensuring cost-effective outcomes and high-quality care.

This role requires an active RN license, 3+ years acute care experience, and the ability to obtain additional state licenses. Typical hours are Mon-Fri 8:00 am–4:30 pm in the residence time zone, with occasional late

Qualifications

• Must hold an active RN license in state of residence.
• Willingness to obtain additional state licenses upon hire (paid for by CVS).
• 3+ years of acute care RN experience required.

Responsibilities

• Apply data-driven methods to identify members for case management or referrals.
• Conduct comprehensive clinical assessments.
• Evaluate needs and develop flexible approaches based on plans and external programs.
• Advocate for patients regarding health care coverage.
• Promote quality, cost-effective outcomes and improve program efficiency.
• Provide high level customer service and coordination to discharge planning and care transitions.
