Job summary

Moda Health in Oregon seeks an RN Case Manager to perform hospital/home health case management within accreditation standards. You will interact with members, families and providers to plan and monitor treatment, with a focus on timely, appropriate care.

This is a full-time work-from-home role in the Government Programs team. Salary range is $71,990.85–$89,988.57 per year, with pay depending on location and qualifications.

Qualifications

• Graduate of accredited school of nursing.
• Current unrestricted Oregon RN license.
• CCMC certification required or obtainable within 24 months of hire.
• 3 years recent hospital, home health or acute care experience.
• Proficient with Microsoft Office applications and 35 wpm typing.
• Strong analytical, problem solving, memory retention, organizational and detail orientation skills.
• Exceptional verbal and interpersonal communication skills including management of the angry customer.
• Ability to work well under pressure and arrive on time daily.

Responsibilities

• Lead essential activities of case management (assessment, planning, implementation, coordination, monitoring and evaluation).
• Assess member situations to identify needs and develop care plans.
• Plan objectives and actions to meet member needs identified in assessment.
• Coordinate resources and monitor plan effectiveness and adherence to standards.
• Consult with physician advisors for clinically appropriate determinations.
• Document consistently, accurately and timely while protecting confidentiality.
• Collaborate with departments to resolve claims, quality of care, and provider issues.
