Job summary

Moda Health is seeking a dedicated care coordination professional to ensure timely, appropriate care for members. You will interact with members, families, and providers to develop and monitor treatment plans and manage utilization reviews.

The role is a full-time, work-from-home position offering comprehensive benefits, with core hours 7am–5:30pm and a focus on high-quality patient outcomes and collaboration across departments.

Qualifications

• Graduate from a nursing program with active RN license
• Holds current unrestricted Oregon RN license
• 3 years of recent hospital, home health, or acute care experience
• Proficient with Microsoft Office and typing 35 wpm
• Strong communication and organizational skills

Responsibilities

• Care coordination within policy to ensure timely, appropriate care
• Develop, coordinate, and monitor the member's treatment plan with families and providers
• Maintain phone coverage during core hours and document cases accurately
• Perform Utilization Review and Service Authorizations for assigned employer groups
• Collaborate with physician advisors and internal departments to support member outcomes
