Job summary

Humana Inc. is seeking a Telephonic Registered Nurse Care Manager to work remotely in a part-time capacity (20 hours/week).

The role supports patients transitioning to post-acute services and collaborates with the CenterWell Home Health clinical team to coordinate home health, hospice, and palliative care. Responsibilities include determining appropriate post-acute care, coordinating referrals, and ensuring timely follow-up while maintaining compliance with HIPAA and company policies.

Qualifications

• Associates Degree required; BSN preferred.
• Compact state RN license required.
• At least 3 years post-acute nursing experience; home health or hospice preferred.
• Knowledge of home health, hospice, and palliative care services.
• Strong communication skills and empathy on the phone.
• Able to train on weekdays evenings and weekends during the day/evening.

Responsibilities

• Be a CenterWell Home Health representative supporting patients discharged from service or needing post-acute services.
• Provide patient options and education using various tools; handle location lookups and inquiries.
• Make clinical level of care determinations and refer to appropriate service sites.
• Support department performance objectives and patient satisfaction goals.
• Assist admissions and act as ambassador for patients meeting admissions requirements.
• Place the right patient to the right care setting at the right time.
• Comply with HIPAA, ethics, and company policies.
