Qualifications

• RN licensure required.
• Bachelor's degree in Nursing or nursing degree from an accredited school.
• 2–4 years of related experience in nursing or care management.

Responsibilities

• Evaluates members' needs and barriers to care and recommends/facilitates the care plan.
• Develops ongoing care plans and coordinates with providers to address needs.
• Identifies problems to care and provides appropriate interventions.
• Coordinates between member/family and care team for person-centered care.
• Provides follow-up and revises care plans to meet goals.
• Offers resources to members and care managers for local services.
• Facilitates care management to ensure timely access to care.
• May conduct telephonic, digital, home, or other outreach.
• Collects and maintains member data to ensure regulatory compliance.
• Educates members and families on disease processes and care options.
• Provides feedback to leadership on opportunities to improve delivery.
