Job summary

CVS Health is seeking an experienced ICM Case Manager RN to join our team remotely with 25-50% travel within Essex County and surrounding areas. The role focuses on assessing and coordinating care for dual eligible Medicare/Medicaid members, addressing complex health and social determinants.

You will develop proactive care plans, collaborate with clinicians and community resources, and use clinical judgment to reduce risks and barriers while promoting overall wellness.

Qualifications

• Minimum 3+ years of clinical practice experience.
• Active and unrestricted RN licensure in the state of NJ.
• Willing and able to travel 25-50% of time using own vehicle.
• Must reside in Essex County, NJ or within 50 miles of Essex.

Responsibilities

• Assess, plan, implement, and coordinate all case management activities.
• Develop proactive plan of care to enhance short and long-term outcomes.
• Collaborate with members, providers, and community resources for wellness.
• Conduct holistic assessments using various information sources (claims, etc.).
• Utilize motivational interviewing to engage members and address health needs.
