Job summary

CVS Health is seeking an experienced Integrated Care Management (ICM) Care Manager to support Medicare/Medicaid members. This work-from-home role involves 25–50% travel within Passaic County and nearby areas; you will assess, plan, implement and coordinate case management to improve member wellness.

The position requires an active NJ RN license, 3+ years of clinical practice, and residence in Passaic County.

Qualifications

• Minimum 3+ years of clinical practice experience.
• Must have active and unrestricted NJ RN license.
• Willing to travel 25-50% using own vehicle; mileage reimbursed.
• Must reside in Passaic County, NJ.

Responsibilities
