Job summary

CVS Health seeks an Integrated Care Management (ICM) Care Manager to assess, plan, and coordinate care for Medicaid/Medicare populations. You will work with members, providers, and community partners to address complex health and social needs while maintaining a proactive care plan.

Role requires active NJ RN license, 3+ years of clinical practice, and 25-50% travel. This is a full-time remote-friendly position within Essex County, NJ with competitive benefits and CVS Health bonus eligibility.

Qualifications

• Minimum 3+ years of clinical practice experience.
• Active and unrestricted RN licensure in the state of NJ.
• Must reside in Essex county, NJ and willing to travel 25-50%.
• Bachelor's Degree in Nursing is preferred; Associate's Degree required.

Responsibilities

• Assess, plan, implement, and coordinate all case management activities for members.
• Coordinate with members, providers, and community organizations to address health and social needs.
• Use clinical tools and data to evaluate member needs and benefits.
• Apply clinical judgment to reduce risk factors and barriers in care planning.
• Collaborate with supervisors and stakeholders in member healthcare to achieve goals.
• Maintain compliance with regulatory and company policies; utilize motivational interviewing to engage members.
