Job summary

CVS Health seeks an experienced ICM Case Manager to join our remote team serving members with Medicare and Medicaid needs. You will develop proactive care plans, coordinate with providers and community resources, and support dual-eligible members across a wide range of health and social challenges.

Strong clinical background and NJ RN license are required; 25–50% travel within Middlesex County area; 40-hour workweek. Work from home with dedicated space and adherence to HIPAA guidelines.

Qualifications

• Active NJ RN license in good standing.
• Minimum 3+ years of clinical practice.
• Willing to travel 25–50% using own vehicle.

Responsibilities

• Evaluate member needs using data and clinical tools.
• Develop care plans to address risks and barriers.
• Coordinate with providers, members, and care teams.
• Document cases and present at interdisciplinary conferences.
• Ensure compliance with policies and HIPAA.
