Job summary

HarmonyCares is seeking an RN Nurse Care Manager in the US-MI-Okemos area to oversee high-risk patients, coordinate care transitions, and drive disease and medication education. You will partner with pod leaders to develop tailored care plans and ensure timely follow-ups, leveraging strong nursing and communication skills.

The role offers a remote work arrangement with a salary full-time position and quarterly bonuses.

Qualifications

• Active RN license is required.
• 2+ years of care management experience in community, health plan or hospital systems.
• Strong clinical and proactive thinking skills.
• Excellent written and oral communication.
• Familiarity with Medicare regulations and home care/hospice standards.

Responsibilities

• Coordinate care services with pod leader to ensure comprehensive services for patients.
• Develop and manage personalized care plans and address care gaps.
• Coordinate transition of care to prevent avoidable hospital admissions.
• Lead High Risk Huddles and ensure follow-up actions are completed.
• Regularly update patient care plans and document interactions in medical records.
