Qualifications

Active Georgia RN license.
Bachelor’s in Nursing or 2+ years geriatric/home-care experience.
Solid clinical assessment skills and problem-solving.
CPR/First Aid certification and valid driver’s license.
Warm, professional communication with families and field staff.

Responsibilities

Assess & Plan – in-home RN assessments; craft individualized Care Plans with client/family approval.
Monitor & Document – review daily care logs; track condition changes and interventions in HIPAA-secure EMR.
Collaborate & Advocate – coordinate with physicians, therapists, and schedulers to keep care seamless and client-centered.
