Perfect-Care-Nursing in Marietta, GA is seeking an experienced Home Health RN to develop and oversee patient care plans, reassess needs, and coordinate with physicians and families. You will train staff, document progress, and ensure safety and regulatory compliance in home health settings.

The role requires 1–3 years of home care nursing, CPR certification, and a proactive, self-directed approach with excellent communication. Travel to patient homes and adherence to policies are essential.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN license in Georgia.
• CPR certification.
• 1–3 years of nursing experience; home care preferred.
• Supervisory skills a plus.
• Excellent communication skills; flexible and positive interaction with staff.
• Strong initiative and urgency in addressing patient crises.
• Self-directed, able to work without immediate supervision.
• Commitment to regulatory compliance and timely documentation.
• Ability to travel and possess valid driver’s license and auto insurance.

Responsibilities

• Develop and implement the service plan for each patient and reassess regularly.
• Participate in in-service training and teach other personnel.
• Observe and report symptoms, treatment responses, and changes in patient condition.
• Initiate preventative procedures for patient safety and environmental changes.
• Maintain clinical notes and progress reports for physicians.
• Coordinate services for patients with other agencies and providers.
• Counsel patients and families in nursing needs.
• Provide supervision of caregiver services and written care instructions.
• Maintain patient confidentiality and timely documentation.
• Support administrators in policy decisions and home care operations.
• Participate in surveys, studies and special projects as assigned.
• Observe attendance and dress code policies; manage time records.
