Job summary

FreedomCare is hiring a Care Manager (RN) to join our Georgia team. This field-based role requires frequent in-home visits to patients in Jonesboro, Medonogh, Lawrenceville and Moreland counties.

You will assess needs, develop plans of care, and coordinate services with interdisciplinary teams. Ideal candidates hold a GA RN license and have at least two years of nursing experience supporting adults or geriatrics.

Qualifications

• Active RN license in Georgia.
• Minimum two years of nursing experience with adults/geriatrics.
• Experience with Medicaid waiver programs, care coordination, or clinical escalation preferred.
• Valid driver’s license and reliable transportation.
• Active CPR/First Aid certification or ability to obtain before starting.

Responsibilities

• Conduct 16–20 in-person visits weekly.
• Complete in-person visit documentation the same business day.
• Educate patients, caregivers, and internal teams on care needs and safety.
• Coordinate patient care with clinical teams and case management partners.
• Ensure HIPAA compliance and field safety protocols.
