Job summary

CareTalk Health is seeking an experienced Registered Nurse to join our fully remote care management team. This role focuses on care navigation, AWV chart preparation, and multi-program care coordination on an evening and overnight schedule.

Ideal candidates have 3+ years of nursing experience, telehealth exposure, and strong documentation skills. Remote work from the U.S. with a private home workspace is required.

Qualifications

• Must hold active, unrestricted RN license in a compact state.
• Experience delivering care in a telehealth or virtual setting.
• Strong documentation and communication skills.
• Ability to work independently in a remote environment.

Responsibilities

• Prepare AWV charts and summaries for CMS compliance.
• Provide telephonic and portal-based care management for complex patients.
• Coordinate orders, referrals, and follow-ups across programs (CCM, RPM, RTM, APCM).
• Maintain accurate patient records and ensure HIPAA compliance.
• Engage patients using motivational interviewing and education.
