Qualifications

• Associate's degree required.
• Current, unrestricted NY RN license.
• 4+ years in community health care.
• 2+ years in long-term care / home health / hospice.
• 1+ year in community case management.
• Must have NY state ID.

Responsibilities

• Assess, plan and implement care strategies tailored to each member.
• Use the UAS-NY to manage the Person Centered Service Plan.
• Serve as single point of contact for the member across continuum of care.
• Communicate required health information to stakeholders for coordinated care.
• Advocate for members and families to reflect needs and choices.
