Qualifications

• Associate's degree is required.
• Current, unrestricted RN license for the state of New York.
• 4+ years of experience working within the community health setting in a health care role.
• 2+ year of experience in long-term care, home health, hospice, public health or assisted living.
• 1+ years of relevant community case management experience coordinating care for individuals with complex needs.
• Must hold active New York state ID.

Responsibilities

• Assess, plan and implement care strategies individualized by member and directed toward the least restrictive level of care.
• Utilize the UAS-NY to manage the Person Centered Service Plan that meets the member's needs.
• Manage the person centered service plan throughout the continuum of care as a single point of contact.
• Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members.
• Advocate for members and their families as needed to ensure the member's needs and choices are fully represented and supported by the health care team.
