Humana Inc. is seeking a Registered Nurse to join the Wisconsin Care Coaching team, coordinating member care, providing education, and helping members navigate their care with confidence.

The role is remote with travel up to 40%; requires an Associate degree in nursing and Wisconsin RN license, plus care management experience preferred. Hours are Monday through Friday, with day shifts and field visits as needed.

Qualifications

• Associate degree in nursing and Wisconsin RN license in good standing.
• Ability to travel ~40% and work from home.
• Experience in care management preferred.

Responsabilités

• Assess members and develop a comprehensive MCP.
• Conduct regular health assessments.
• Coordinate with acute and primary care providers.
• Educate members on diseases, nutrition, exercise and lifestyle changes.
• Document in member files per DHS timeframes.
• Monthly member contacts and in-person visits.
• Travel to member homes as needed (up to 40%).
