Job summary

In this role, you will build therapeutic relationships, manage care transitions, perform medication administration, conduct telephone triage, document in the EMR, and support the clinical team with care coordination.

Qualifications

• Graduate of a State-approved school of nursing.
• Current, unrestricted license to practice professional nursing in the Commonwealth of Massachusetts.
• American Heart Association Basic Life Support (BLS).
• Computer experience with word processing and spreadsheets.
• Electronic medical record (EMR) experience or aptitude to master EMR.

Responsibilities

• Build therapeutic and supportive relationships with patients and families, providing education and managing care transitions.
• Deliver patient care and follow-up in alignment with RN protocols, including medication administration.
• Conduct telephone triage, elevate concerns appropriately, and support the clinical team with care coordination.
