Job summary

North Memorial Health is seeking a Care Manager to coordinate clinically-based care across primary care and specialty clinics. You will ensure optimal care for complex cases, including acute and chronic disease management, across multiple health settings and physicians.

The role requires RN licensure or MSW background, 5 years of care coordination experience, and strong communication. Remote work option with standard day shifts may apply.

Qualifications

• Graduate from an accredited school of nursing or social work program as described in the job requirements.
• RN licensure or social work licensure required depending on track.
• Experience in care coordination and utilization management preferred.

Responsibilities

• Provide clinically-based care coordination to support quality customer care.
• Coordinate care with primary care providers and clinic care teams.
• Develop care plans and monitor outcomes across diverse settings.
• Apply assessment, planning, implementation, monitoring and evaluation steps.
• Engage customers and families to activate and sustain care plans.
• Demonstrate knowledge of utilization management concepts.
