Job summary

Evry Health Inc. is seeking an RN Integrated Care Coordinator to deliver comprehensive care coordination for our fully insured commercial population.

This remote role emphasizes proactive member engagement, individualized care planning, and utilization review support in a technology-forward environment. You will develop care plans, educate members and providers, and document all activities while promoting Evry Health’s digital health programs.

Qualifications

• RN diploma and active license in Texas or compact license.
• 3–5 years direct patient care experience, plus 1–2 years in health plan/MBU.
• Familiarity with CPT/ICD-10, authorizations, and pre-certification processes.
• Bilingual English/Spanish required for member interactions.

Responsibilities

• Proactive and reactive member engagement based on risk data.
• Develop and monitor individualized care plans and coordinate resources.
• Educate members and providers on benefits and self-management.
• Serve as liaison between UM team, providers, and members.
• Document activities, manage escalations, and promote digital health programs.
