Job summary

Community Health Center of Southeast Kansas, Inc. is seeking a Nurse Care Coordinator to provide remote nursing care within the EHR system and liaise between providers and patients regarding test results and care continuity.

The role supports the clinical team, coordinates upcoming appointments, triages patient calls, and ensures documentation and follow-up are completed in a timely, patient-centered manner.

Qualifications

• Active RN license in the practicing state or multistate license.
• Minimum two years of outpatient clinic nursing experience.
• BLS certification within six months of hire.
• Proficient with EHR systems and MS Office; maintains patient confidentiality.

Responsibilities

• Manage EHR tasks, telephone encounters, documents, and lab/diagnostic orders.
• Coordinate care around appointments and immunization status; ensure records are current.
• Triage calls and act as primary contact for Mission Control and care coordination.
• Support the care team to maintain continuity of care and patient communication.
